Family Service League is currently accepting applications for employment with two new programs beginning in June and September.
FSL’s new Crisis Stabilization Center (CSC) will offer 24/7 outpatient services to individuals who are feeling overwhelmed by mental health, substance abuse, or other related stressors. Services will include assessments, counseling (individual and/or family), Peer Advocacy and supports, and linkage to community based services. This program will work with our 24/7 hotline (also expected to begin in September, 2018) and in coordination with our new Mobile Crisis Teams.
FSL’s new Mobile Crisis Team (MCT) will respond rapidly to a mental health crisis in the community. The goal is to engage individuals in crisis and, when applicable, divert use of hospitals and emergency rooms. A team will respond to the individual or family in their home, place of employment, school, etc. Assessments, crisis intervention, and linkage to other appropriate services for aftercare will be provided and coordinated.

Homeless Services Care Coordinator (2017-397 & 398)

SUMMARY:
Family Service League is hiring two Homeless Services Care Coordinators/Care Navigators who will be out-stationed at select shelters and other settings that serve the homeless population. These Navigators will be responsible for screening individuals to determine their behavioral health, concrete needs and relevant service eligibility. She/he will also provide linkages to services for which individuals in homeless settings may be eligible, such as: Health Home, HARP enrollment, SPA housing, clinics, etc. The goal of this position is to improve the prospects of stable, permanent housing by increasing access to services and benefits that will help facilitate housing placement and better behavioral and medical health outcomes.

RESPONSIBILITIES:
• Perform work at homeless service settings both within and outside of FSL programs.
• Conduct clinical evaluations and program screenings to identify needs and eligibility for an array of services including, Health Home, HARP, SPA Housing, behavioral health services, etc.
• Demonstrate engagement skills, an understanding of a variety of assessment and program eligibility screening tools, and relevant resources to assist the target population.
• Form and maintain a positive and professional relationships with both internal and external providers in order to coordinate care, obtain needed information, and facilitate linkages to appropriate benefits and services.
• Employ a culturally sensitive and proactive approach to all members.
• Complete all required documentation.
• Report all incidents to the program management staff.

QUALIFICATIONS:
Licensed behavioral health professional e.g., LMSW, LCSW, LMHC, LMFT, Psychologist required. The Waiver Program serves youth, between the ages of 5 and 21 who are Severely Emotionally Disturbed and at risk of imminent hospitalization. The Crisis Response Worker will respond either telephonically or in person in accordance with HCBS Waiver regulations and as client needs dictate. The “on call” capacity will be after business hours and on weekends.

RESPONSIBILITIES:
• Perform interventions designed to assist children and families when they are in crisis.
• Provide immediate, short-term interventions until linkages are made with other appropriate services. This may include assessment, consultation, facilitating the safety plan’s interventions, and referral wherever and whenever necessary.

SUMMARY:
Family Service League is seeking a full-time Bi-lingual Care Coordinator to work with youth with mental illness and their families at the Children’s Health Home Program in Mastic Beach. The program provides case management services to severely emotionally disturbed children and adolescents between 5 – 18 years of age, who reside in Suffolk County. Services include individualized goal planning, linking to appropriate services, monitoring and advocacy. The program is community based and the majority of contacts occur in the consumer’s home or community

RESPONSIBILITIES:
• Maintains a minimum of visiting each child/adolescent four to two times per month.
• Maintains regular communication with parent/guardian including meeting with them minimally twice per month.
• Maintain contacts with other service providers to coordinate services.
• Effectively manages a diverse caseload and responds professionally to emergency and crisis situations including additional visits when indicated.
• Works to achieve goals set by children/adolescents and their families.
• Responds to families changing needs including creating additional service plans and facilitating transfers when necessary.
• Maintains case notes within 48 hours of any given contact with a child or on their behalf.
• Follows all OMH regulated time guidelines pertaining to openings, assessments, reviews and CANS.

QUALIFICATIONS:
Fluent in Spanish required. Bachelor’s Degree plus two years of experience or a Master’s Degree plus one year of experience is required. Must have at least 1 year of experience working with the mental health, at risk, case management or related populations. SUMMARY:
Family Service League is seeking a full-time Social Worker to provide weekly in home therapy for the Mobile Clinic based in Huntington.

RESPONSIBILITIES:
• Screen referrals to the Mobile Clinic for appropriateness and maintain waiting list.
• Conduct comprehensive assessments in the family home.
• Provide individual and family therapy to children and adolescents diagnosed with a full range of DSM diagnoses in their home setting.
• Complete all clinical documentation in accordance with NYS Office of Mental Health in an electronic medical record.
• Perform monthly reporting.
• Attend monthly county meetings.
• Attend routine supervision and team meetings.

QUALIFICATIONS:
LMSW, LMHC, LCSW or related licensed clinical degree required. At least 6 months prior clinical experience working with children, adolescents and families required. Strong interpersonal and verbal and written communication skills are required. The ideal candidate will possess knowledge of diagnosing using DSM 5 and experience with behavioral and therapeutic interventions for children and adolescents, as well as families. Proficient computer skills, including Microsoft Office and familiarity with electronic medical records required. Candidate must have reliable transportation, a clean Driver’s License and valid auto insurance. Must be available to work 3-4 evenings per week.

SUMMARY:
Family Service League is seeking a full-time Coordinator, who will identify barriers to service accessibility and implement localized systemic means to improve the quality of services for all clients needing dual recovery services throughout Suffolk County.

RESPONSIBILITIES:
• Identify existing challenges to service accessibility and cooperation and implement solutions for overcoming such challenges.
• Identify workforce training needs and facilitate cross-agency and cross-discipline competency-based staff development.
• Increase awareness of Evidenced Based Practices as it applies to populations with co-occurring disorders.
• Craft a protocol to develop and monitor integrated, client-centered service and treatment plans.
• Craft user friendly guidance documents to support educational activities, resource development and dissemination and operations and policy recommendations.

QUALIFICATIONS:
Master’s Degree in Human Services required; CASAC preferred. At least 1 year of relevant work experience in an administrative capacity and ability to supervise staff members required. Candidate must possess excellent verbal and written communication skills. Proficient computer skills, including Microsoft Office required.
