• Review all referrals and coordinate with priority referral sources. Coordinate an open access process for referrals.
• Work with Information Technology staff to work with the electronic health record and use of other technology tools and computer applications.
• Attend weekly supervision and monthly clinic director meeting with Sr. Director of Clinics. Attend other scheduled meetings and trainings as requested, including off-site meetings.
• Maintain licensure in a relevant clinical field.

QUALIFICATIONS:

Crisis Response Worker P/T (#2017-139)

Family Service League is seeking a per diem Crisis Response Worker for the HCBS Waiver Program to be a part of an On-call Rotation Team. The Waiver Program serves youth, between the ages of 5 and 21 who are Severely Emotionally Disturbed and at risk of imminent hospitalization. The rotation would be once every 4 to 6 weeks for 1 full week (including the weekend). The on-call rotation is paid regardless of whether the individual is called and additional pay is given for actual hours worked. The on call capacity would happen after hours and on weekends, and can be done in conjunction with a full time position.

RESPONSIBILITIES:
Responsibilities include perform interventions designed to assist children and families when they are in crisis and provide immediate, short-term interventions until linkages are made with other appropriate services. This may include assessment, consultation, facilitating the safety plan’s interventions, and referral wherever and whenever necessary.

QUALIFICATIONS:

The program provides case management services to severely emotionally disturbed children and adolescents between 5 – 21 years of age, who reside in Eastern Suffolk County. Services include individualized goal planning, linking to appropriate services, actively communicating with providers, monitoring and advocacy. The program is community based and the majority of contacts occur in the consumer’s home or community.

RESPONSIBILITIES:
Responsibilities include home visiting, maintaining regular and in depth contacts with other service providers to coordinate services, effectively managing a diverse caseload and responding professionally to emergency and crisis situations including additional visits when indicated. Other functions include working to achieve goals set by children/adolescents and their families, responding to families changing needs, including creating additional service plans and safety plans when necessary, maintaining case notes, following all OMH guidelines and leading a team of providers toward the common goal of maintaining youths in their home.
