Housing Specialist

Family Service League is looking for a full-time Housing Specialist for our Family Shelter located in Brentwood.

Responsibilities

Working 35 hours per week, the Housing Specialist is responsible to assist homeless families in obtaining permanent housing. Utilize housing resources to form relationships with landlords and brokers, inspect apartments, accompany clients to apartment viewings, and collaborate with the Department of Social Services (DSS) to link apartments and utilize rental subsidy programs.

RESPONSIBILITIES:

maintaining regular communication with parent/guardian including meeting with them minimally twice per
month, maintaining contacts with other service providers to coordinate services, effectively manage a
diverse caseload and respond professionally to emergency and crisis situations including additional visits
when indicated, working to achieve goals set by children/adolescents and their families, responding to
families changing needs including creating additional service plans and facilitating transfers when
necessary, maintaining case notes within 48 hours of any given contact with a child or on their behalf.

QUALIFICATIONS:
Bachelor’s Degree plus two years of experience or a Master’s Degree plus one year of experience is

The program provides case management services to severely emotionally disturbed children and adolescents between 5 – 18 years of age, who reside in Suffolk County. Services include individualized goal planning, linking to appropriate services, monitoring and advocacy. The program is community based and the majority of contacts occur in the consumer’s home or community.

RESPONSIBILITIES:
Responsibilities include maintaining a minimum of visiting each child/adolescent four times per month, maintaining regular communication with parent/guardian including meeting with them minimally twice per month, maintaining contacts with other service providers to coordinate services, effectively manage a diverse caseload and respond professionally to emergency and crisis situations including additional visits when indicated, working to achieve goals set by children/adolescents and their families, responding to families changing needs including creating additional service plans and facilitating transfers when necessary, maintaining case notes within 48 hours of any given contact with a child or on their behalf. Follows all OMH regulated time guidelines pertaining to openings, assessments, reviews and CANS.
