Working 35 hours per week, the Housing Specialist is responsible to assist homeless families in obtaining permanent housing. Bachelor’s Degree plus two years of experience or a Master’s Degree plus one year of experience is

SUMMARY:
Family Service League seeks a Weekend Supervisor to work in the Brentwood Family Shelter to ensure effective and efficient shelter operation throughout the weekend, provide leadership in directing services and support to all clients and weekend staff, and work in coordination with the security staff to ensure adherence to shelter policies and procedures.
This position is On-call Friday 5:00pm to Monday 9:00am with On-site hours of Saturday 2:00pm-12:00am, Sunday 3:00pm – 11:00pm and weekly supervisors’ meeting.

RESPONSIBILITIES:
• Ensure effective shelter operations.
• Supervise weekend shelter staff.
• Assist clients on intake procedures, admission requirements and FSL policies and procedures.
• Oversees weekend client intakes and unit assignments with FSL staff and security.
• Coordinate unit assignments to maximize shelter utilization.
• Oversee unit inspections.
• Conduct Room (curfew) Checks.
• Manage weekend Pantry. Coordinate client requests and distribution with Case Managers.

• Document all incident reports for shelter, when on-site. Review weekend staff’s incident reporting. Notify, in writing, the appropriate FSL staff of all violations of policies and procedures or operational irregularities.
• Work collaboratively with the Security staff to maintain a safe shelter environment.
• Perform other duties and responsibilities as assigned by Program Director.

QUALIFICATIONS:
Bachelor’s degree required, graduate level preferred. Previous experience working in a homeless shelter is required. Must possess demonstrated proficient computer skills, including MS Office. Family Service League seeks a full-time Care Coordinator to work with youth with mental illness and/or chronic medical conditions and their families in our Bay Shore location. The program provides Care Coordination to children, adolescents and young adults with chronic medical conditions or who are severely emotionally disturbed. The age range is between 5 – 20 years of age, who reside in Suffolk County. Services include individualized goal planning, linking to appropriate services, monitoring and advocacy. The program is community based and the majority of contacts occur in the consumer’s home or community.

RESPONSIBILITIES:
• Maintain a minimum of visiting each child/adolescent two times per month.
• Maintain regular communication with parent/guardian including face to face meetings minimally once per month.
• Maintain contacts with other service providers to coordinate services.
• Effectively manage a diverse caseload and respond professionally to emergency and crisis situations including additional visits when indicated.
• Work to achieve goals set by children/adolescents and their families.
• Respond to families changing needs including creating additional service plans and facilitating transfers when necessary.
• Maintain case notes within 48 hours of any given contact with a child or on their behalf.
• Follow all DOH and OMH regulated time guidelines pertaining to openings, assessments, reviews and CANS.

QUALIFICATIONS:
Bachelor’s Degree plus 2 years of experience or a Master’s Degree with 1 year of experience is required. Must have at least 1 year experience working with the mental health, at risk, case management or related populations. Bilingual (Spanish/English) a strong plus. The DDC identifies barriers to services accessibility and implements localized systemic means to improve the quality of services for all persons needing dual recovery services (Substance Use Disorder & Mental Health Diagnosis) throughout Suffolk County. The DDC will employ a systemic approach to network development that includes cross training of professionals in Substance Use Disorder services to build competency and capacity to serve individuals with co-occurring Mental Health Service needs and, likewise, cross train professionals in Mental Health Services to build competency and capacity to serve individuals with co-occurring Substance Use Disorders, to improve accessibility to services for clients; to promote effective management for the purpose of collaboration and integration of diversified care, to develop client service outcome measurement tools related to those persons assessed as being in need of the widest array of service types.

RESPONSIBILITIES:
• Identify existing challenges to service accessibility and cooperation and implement solutions for overcoming such challenges.
• Identify workforce training needs and facilitate cross-agency and cross-discipline competency-based staff development.
• Increase awareness of Evidenced Based Practices as it applies to populations with co-occurring disorders (TIP 42 & other resources).
• Craft a protocol to develop and monitor integrated, client-centered service/treatment plans.
Craft user friendly guidance documents to support: educational activities, resource development and dissemination, operations/policy recommendations
• Prepare and submit timely progress reports to NYS OASAS.

QUALIFICATIONS:
