Family Service League is currently accepting applications for employment with two new programs beginning in June and September.
FSL’s new Crisis Stabilization Center (CSC) will offer 24/7 outpatient services to individuals who are feeling overwhelmed by mental health, substance abuse, or other related stressors. Services will include assessments, counseling (individual and/or family), Peer Advocacy and supports, and linkage to community based services. This program will work with our 24/7 hotline (also expected to begin in September, 2018) and in coordination with our new Mobile Crisis Teams.
FSL’s new Mobile Crisis Team (MCT) will respond rapidly to a mental health crisis in the community. The goal is to engage individuals in crisis and, when applicable, divert use of hospitals and emergency rooms. A team will respond to the individual or family in their home, place of employment, school, etc. Assessments, crisis intervention, and linkage to other appropriate services for aftercare will be provided and coordinated.

Homeless Services Care Coordinator (2017-397 & 398)

Family Service League is hiring two Homeless Services Care Coordinators/Care Navigators who will be out-stationed at select shelters and other settings that serve the homeless population. These Navigators will be responsible for screening individuals to determine their behavioral health, concrete needs and relevant service eligibility. She/he will also provide linkages to services for which individuals in homeless settings may be eligible, such as: Health Home, HARP enrollment, SPA housing, clinics, etc. The goal of this position is to improve the prospects of stable, permanent housing by increasing access to services and benefits that will help facilitate housing placement and better behavioral and medical health outcomes.

RESPONSIBILITIES:
• Perform work at homeless service settings both within and outside of FSL programs.
• Conduct clinical evaluations and program screenings to identify needs and eligibility for an array of services including, Health Home, HARP, SPA Housing, behavioral health services, etc.
• Demonstrate engagement skills, an understanding of a variety of assessment and program eligibility screening tools, and relevant resources to assist the target population.
• Form and maintain a positive and professional relationships with both internal and external providers in order to coordinate care, obtain needed information, and facilitate linkages to appropriate benefits and services.
• Employ a culturally sensitive and proactive approach to all members.
• Complete all required documentation.
• Report all incidents to the program management staff.

QUALIFICATIONS:
Licensed behavioral health professional e.g., LMSW, LCSW, LMHC, LMFT, Psychologist required. RESPONSIBILITIES:
Responsibilities include assisting the teacher with all aspects of the management of the classroom, working with children 1:1 and in small groups. Family Service League is seeking a Crises Response Worker to provide 24 hour on call intervention, in an on call capacity, as part of the Waiver Team. The Waiver Program serves youth, between the ages of 5 and 21 who are Severely Emotionally Disturbed and at risk of imminent hospitalization. The Crisis Response Worker will respond either telephonically or in person in accordance with HCBS Waiver regulations and as client needs dictate. The “on call” capacity will be after business hours and on weekends.

SUMMARY:
The Peer Specialist is part of the mobile crisis team that will be housed at our at our 24 hour per day, 7 day per week crisis stabilization center. The Peer Specialist will assist with the outreach, support and coordination of care for clients treated in this program. Day, evening, and overnight shifts available.

RESPONSIBILITIES:
• Engage individuals in crisis in the community and provide follow-up services
•Provide crisis intervention services, at the direction of professional staff, in team based approach to individuals who are feeling overwhelmed with mental health and related issues.

SUMMARY:
Family Service League is seeking a full-time Program Director for our Children’s Care Coordination Program. The Children’s Care Coordination Program is a home visiting program that offers care coordination to children and young adults with mental illness and/or chronic medical conditions. The Director is responsible for general oversight and leadership of the program.

RESPONSIBILITIES:
• Oversee a staff of 20+ Care Coordinators and provide direct supervision to four Program Coordinators and a Program Assistant
• Handle recuitment, training, performance management and time reporting for staff
• Oversee billing protocol and fiscal management
• Conduct chart reviews
• Ensure adherence to program, regulatory and COA standards
• Remain current on system changes and educate staff
• Interface with children’s SPOA, DOH, Health Home/s and other community entities
• Interview, hire, evaluate and train staff
• Monitor and review staff vehicle use
• Attend meetings as appropriate
• Generate timely reports
• Identify and develop strategies to assist high risk clients
• Conduct community outreach
• Monitor service dollar expenditures
• Asist with after hour crisis as needed
• Perform other tasks as assigned

QUALIFICATIONS:
Master’s degree in Social Work or related human service field required. LMSW, LCSW, LHMC or related license preferred.
Five years’ of related experience, including strong knowledge of care management/care coordination and experience with youth and or families required.
At least three years of prior supervisory experience required.
Must possess excellent clinical, judgement, leadership, time management, interpersonal, organizational, and verbal and written communication skills.
Proficient computer skills, including Microsoft Office, Excel and Word and electronic health record.
NYS driver license required; must be willing to travel as needed to different FSL offices.

SUMMARY:
Family Service League is seeking a part-time Case Manager for our Community Home Base program, to work with youth with probation involvement and their families, to help prevent out of home placement.

RESPONSIBILITIES:
• Provide linkage, coordination, advocacy, mentoring, and anger management and parenting skills classes training to youth and their families.
• Provide services that focus on goal achievement surrounding the factors that contributed to probation involvement.
• Conduct home visits throughout Suffolk County.

QUALIFICATIONS:
Associates Degree required; Bachelor’s degree preferred.
At least 1 year of experience working with families in case management, or related populations, as well as knowledge of community resources required.
Must possess excellent verbal and written communication skills, sound judgement, reliability, flexibility, and the ability to multitask.
Demonstrated ability to work with diverse populations and engage both youth and parents in service delivery.

SUMMARY:
As a member of the Finance Department, the Financial Analyst will be an integral part of monthly, quarterly and annual analysis, reporting and other financial oversight responsibilities involved with working at a nonprofit organization.

RESPONSIBILITIES:
•Handle grant and contract claims to various funding sources
•Communication and follow-up regarding disallowances on claims

QUALIFICATIONS:
•Bachelor’s degree in Accounting, Business, Finance or related field required.
•Two year’s related experience as a Financial Analyst required. Prior cost accounting, budget, analysis and reporting experience preferred. Non-profit experience also preferred.
•Proficient computer experience, including MS Office; advanced Microsoft Excel skills required. Knowledge of databases and familiarity with accounting software packages preferred
•Demonstrated ability to extract, analyze, summarize and present data.

The Healthy Families New York home visiting program matches parents with knowledgeable and caring workers who provide information and support during pregnancy and early childhood. Services include helping families access community resources and services, educating families on parenting and child development, connecting families with medical providers, and assessing children for developmental delays.

RESPONSIBILITIES:
• Initiate and maintain regular contact with families, primarily in their homes
• Establish a trusting relationship with families
• Provide prenatal and other health education
• Assist in strengthening parent-child relationships
• Assist parents in improving their skills to optimize the home and environment for the child’s well-being and safety
• Assist in making and attending health and human service appointments, including activities related to employment and educational goals
• Travel to different locations for training purposes

QUALIFICATIONS:
High school diploma or GED required
Experience working with at-risk children and families in a community setting, and successful parenting and/or child care experience is preferred.
Knowledge of infant and child development, infant mental health endorsement Level I or II preferred

SUMMARY:
The Housing Specialist reports to the Program Director and is responsible for assisting homeless families in obtaining permanent housing. The Housing Specialist will develop relationships with landlords and brokers, inspect apartments, accompany clients to apartment viewings, and collaborate with government agencies to link apartments and utilize rental subsidy programs.

RESPONSIBILITIES:
•Conduct comprehensive intakes of all clients
•Develop individualized housing plan for each client
•Locate appropriate housing for households who are homeless
•Document all contact with and on behalf of the program participants
•Ensure timely completion of housing inspections
•Arrange forums with outside providers to provide information on topics related to securing and maintaining permanent housing
•Provide additional information/instruction on the skills required to secure and maintain permanent housing to program participants
•Assist participants in the completion of all housing applications and ensure the applications are submitted to housing programs in a timely manner
•Develop new housing resources
•Escort participants to view apartments and other appointments as needed
•Assist participants in moving into permanent housing e.g. attends lease signing, ensures utilities are turned on, and coordinates transportation and moving
•Participate in team meetings, case conferences, department meetings, weekly individual supervisory sessions, and staff training
•Document and maintain up to date records on services provided
•Perform other duties as assigned

QUALIFICATIONS:
•Associates Degree in a related field required.
•Prior experience in case management, housing resources, assessment, counseling, and crisis intervention preferred.
•Knowledge of the challenges faced by the homeless population and the supportive services available to successfully achieve the goal of permanent housing and self-sufficiency preferred.

•Conduct vocational assessments for all clients.
•Meet regularly with clients to evaluate their interests, qualifications, and limitations to assist them in developing appropriate employment objectives.
•Review and assess educational and social information concerning clients.
•Help clients understand their capabilities and develop appropriate career goals.
•Document individual vocational plans and maintain required records.
•Assist clients to develop or reestablish skills, attitudes, personal characteristics, interpersonal skills, work behaviors, and functional capacities to achieve employment outcomes.
•Refer clients and act as liaison to training/educational programs.
