• Ensure compliance with Agency and state initiatives including participation in the PSYCKES initiative; quality management projects; review and tracking of statistics related to productivity and performance indicators as well as clinic-wide outcome measures.
• Review all referrals and coordinate with priority referral sources. Coordinate an open access process for referrals.
• Work with Information Technology staff to work with the electronic health record and use of other technology tools and computer applications.
• Attend weekly supervision and monthly clinic director meeting with Sr. Director of Clinics. Attend other scheduled meetings and trainings as requested, including off-site meetings.
• Maintain licensure in a relevant clinical field.

QUALIFICATIONS:
LCSW, Ph.D./PsyD or licensed professional in a related field required. Healthy Families is a home-visiting based service for high-risk families of newborns who reside in: Amityville, Wyndanch and Copiague communities. Crisis Response Worker P/T (#2017-139)

SUMMARY:
Family Service League is seeking a per diem Crisis Response Worker for the HCBS Waiver Program to be a part of an On-call Rotation Team. The Waiver Program serves youth, between the ages of 5 and 21 who are Severely Emotionally Disturbed and at risk of imminent hospitalization. The rotation would be once every 4 to 6 weeks for 1 full week (including the weekend). The on-call rotation is paid regardless of whether the individual is called and additional pay is given for actual hours worked. The on call capacity would happen after hours and on weekends, and can be done in conjunction with a full time position.

RESPONSIBILITIES:
Responsibilities include perform interventions designed to assist children and families when they are in crisis and provide immediate, short-term interventions until linkages are made with other appropriate services. This may include assessment, consultation, facilitating the safety plan’s interventions, and referral wherever and whenever necessary.

RESPONSIBILITIES:
•Develop new community coalitions to prevent underage drinking and substance abuse.
•Attend & facilitate existing coalitions’ meetings.
•Initiate or facilitate five activities quarterly which will aid in the recruitment of stakeholders for the purpose of coalition capacity building.
•Develop and launch public information campaigns to educate, promote and foster community health & wellness initiatives.
•Cooperatively address substance abuse and the consequences of use and abuse in the coalition communities in collaboration with existing treatment & prevention providers and community leaders.
•Provide technical assistance in the planning and implementation of environmental strategies consistent with the needs of the community.
•Develop an action plan, a logic model and communication plans for coalitions.

QUALIFICATIONS:

SUMMARY:
