Employee benefits at FSL
Full-time employees and staff working 21 hours and over are eligible for benefits after working for three full months. Benefits for part-time staff are pro-rated.
MEDICAL AND DENTAL COVERAGE:
Family Service League (FSL) provides single coverage for employees for both medical and dental at a rate of 90%. Staff pays for the cost of family coverage in addition to their 10% contribution towards individual coverage. We currently offer Empire, and HIP. Guardian is our dental carrier.
LIFE INSURANCE:
FSL provides life insurance through Aetna Life Insurance Company at no cost to employees, equal to one year's salary. We also offer additional voluntary life insurance through Mass Mutual.
SHORT AND LONG-TERM DISABILITY:
FSL provides both short-term and long-term disability at no cost to employees through Zurich American Insurance Company (STD) and Aetna Life Insurance Co. (LTD).
AFLAC:
FSL offers supplemental, voluntary insurance plans through AFLAC including: cancer and accident protection, and additional dental and vision. These are offered to all FSL staff. Additional short-term disability and life insurance is open to full-time staff that are working over 30 hours/week. All staff contributions for insurance are deducted from payroll and are pre-taxed except short term disability and life insurance plans which are after tax.
PENSION AND TDA:
FSL provides pension coverage and voluntary contributions to a Tax Deferred Annuity (TDA) through Mutual of America. Staff electing not to receive medical coverage receive $1,800* ($450 deposited each quarter) to a TDA. Pension coverage is 5% of salary-eligible after 1 year with 3 years to vesting.

TIME BENEFITS:
VACATION:
Full-time staff receives 4 weeks vacation and are eligible after a 6-month introductory period has been satisfactorily completed. Eligible part-time staff receive pro-rated time.
HOLIDAYS:
Full-time staff receive twelve holidays for the year.
PERSONAL DAYS:
Full-time staff receive 4 personal days annually and they may not be carried over.
SICK:
Full time staff receive 7 sick days annually, and may accumulate a maximum of 30 days.
