Supportive Case Manager for Adult Case Management program in Bay Shore. Assist in linking consumers to services and resources in mental health, housing, vocational, education, legal, health, and financial areas. BA or AA with minimum two years experience in mental health or closely related field. Experience can include volunteer work and internships. On-going communication with providers to coordinate services. Organizational, time management, writing, computer, and engagement skills a must. Knowledge of recovery-based case management a plus. Grants Coordinator who will work with staff to prepare grants and develop programs. Must have experience in writing government, corporate, and foundation proposals, as well as ability to research and identify resources which support the work of the organization. Must have BA or MA in related field, strong writing skills, able to work as a team member, three- five years demonstrated experience in grant writing and program development with other NFP’s.
