Adult Health Home provides Health Home and State Aid Care Management services for adults with chronic mental health, medical, and substance abuse disorders for the purpose of increasing community longevity and connections while decreasing the need for inpatient and emergency room visits.

The Assertive Community Treatment Team (ACT Team)

The Assertive Community Treatment Team (ACT Team) delivers comprehensive therapeutic, rehabilitative, and supportive services to individuals diagnosed with severe mental illness, and whose needs have not been sufficiently met by more traditional service delivery systems. These valuable services are provided by an interdisciplinary team situated within the community.

The Autism Social Skills Program

The Autism Social Skills Program offers a variety of services to those whose lives are touched by a child diagnosed with autism.  This program also helps parents and siblings meet challenges within the family by providing parent and sibling support groups.

The Autism Respite Recreation Program

The Autism Respite Recreation Program enhances the social skills of autistic children in a relaxed, recreational setting. For these children, who are often excluded from peer playgroups, this program prevents social isolation. Siblings are encouraged to participate, as well as parents.

The Children and Parents Together (CAPT)

The Children and Parents Together (CAPT) program helps parents with mental illness sustain mental health quality, function better personally, as a parent, and outwardly towards the community.  An important aspect of this treatment includes ongoing infant/toddler stimulation and age-appropriate activities for their children, as well as active participation in their child’s preschool education, socialization, and skills development.

Community Behavioral Health Collaborative (CBHC)

The Community Behavioral Health Collaborative (CBHC) works with East End communities to raise awareness and sensitivity about mental illness, create linkages for assistance, provide education, and strengthen family support.

CBHC will host educational workshops for professionals, lay people and children on issues impacting emotional well-being in children, including suicide prevention. Selected families in the Springs School District will meet weekly for 8 week sessions and participate in family strengthening exercises, a parent support group and programs for youth. Social workers provide referrals and information to east end families struggling to reach their goals.

Diagnostic, Assessment & Stabilization Hub (DASH)

The Diagnostic, Assessment & Stabilization Hub (DASH) program prevents individuals who are feeling overwhelmed due to substance use, mental illness and/or other life stressors from unnecessary utilization of emergency room visits/ inpatient hospitalization. Includes the Mobile Response Team, the Crisis Hotline, and the Crisis Stabilization Center.

The DASH crisis stabilization program sits at the center of a multi-pronged approach to crisis service delivery which includes a 24/7 crisis hotline and mobile response team. The program includes rapid assessment, triage, stabilization, sobering, coordination of care, and linkage to longer-term outpatient services or, when necessary, to a higher level of care. Length of stay is up to 23 hours and 59 minutes.
The Mobile Crisis Team provides crisis services in the community where the person is experiencing a crisis. Will engage individual and/or family members, conduct risk assessment, develop a safety plan, and divert use of hospitals and emergency rooms. When applicable, link to necessary services and supports. Teams consist of licensed professional staff and certified peers.
Crisis Hotline (anticipated start date Nov. 2018) staff will provide telephone triage, clinical assessments, and therapeutic support. Appropriate referrals will be made which may include outpatient clinic services, mobile crisis response, crisis stabilization, and support services.
Crisis Stabilization Center (anticipated start date Feb 2019) will serve Suffolk County adults of all ages and children five years of age and older who are experiencing a behavioral health crisis or family issues but do not require hospital emergency room services. The Stabilization Center will serve as an alternative to an emergency department and is 100% voluntary. An interdisciplinary team of professionals, paraprofessionals, and support staff will focus on early intervention, crisis resolution, observation (up to 23 hours and 59 minutes), and linkage to treatment and concrete support services in the community.

Health Home / Children and Adolescents Care Coordination provides case management services to children, adolescents and their families in order to maximize each youth’s potential for growth and emotional stability within their natural support system and maintain the youth in their home and community.

Home and Community Based Services (HCBS) Children’s Waiver Program

The Home and Community Based Services (HCBS) Children’s Waiver Program’s purpose is to prevent long term psychiatric hospitalization and out of home placement for youth who are living in the community and have significant mental health needs that cannot be met by traditional community programs.

Joe’s Project

Joe’s Project provides immediate and ongoing psychological support to those who are affected by, or have contemplated, suicide. This important program also works within the community, organizing school functions to provide educational awareness and promote suicide prevention.

LEAD (Learn Enrich Achieve Dream)

LEAD provides therapeutic recreation for youth with serious emotional disturbances ages 14-18.

LEAD fosters leadership skills in youth. Participants will be exposed to a variety of learning experiences to improve communication and problem solving skills. Youth are assisted by staff and peers to cope with emotional and social challenges.

Mental Health Clinics and Satellites

Mental Health Clinics and Satellites employ evidence-based treatment methods applied through multiple clinical specialties.  Various treatment modalities are used, as well as supportive services, including advocacy and referral, entitlement assistance, and other community sponsored self-help programs. The goal is to reduce mental health symptoms, strengthen and build self-sufficiency, and improve quality of life by providing expert, professional services to those living with mental illness.

RECESS Therapeutic Recreation

RECESS is a volunteer program that provides therapeutic recreation to children ages 7-14 with serious emotional disturbances. Using group interventions, education, and skill building, this program helps children improve functional responsibilities, including age-appropriate impulse control, socialization skills, motor coordination, and tolerance for frustration.
