Family Service League appreciates that comprehensive health care for individuals with mental health needs must take a collaborative approach— health care providers and care management services working together to achieve true integrated health care. This philosophy is born out in our Community Health Care Collaborative (CHCC), an FSL program that has successfully coordinated a support system of primary care providers working directly with mental health professionals.
Community Health Care Collaborative (CHCC)
This program was developed in 2011, in collaboration with Northwell Health at Southside Hospital— a partnership that was created in order to provide community-based medical and care management services at our Bay Shore Family Center. Exceeding our expectations, this program has filled a significant need in providing access to primary health care for many of our clients with mental illness, as well as referrals to specialty medical services as needed.
Co-Locations with Medical Practices
In order to further its mission of providing integrated care, FSL currently co-locates licensed social workers in four primary care practices on Eastern Long Island. These physician practices have fully embraced the model, in part, because primary care settings are often the gateway for many individuals with behavioral health needs. This successful prototype provides a seamless, collaborative, caring process that results in a safe, effective and timely care regimen for individuals, in a primary care setting, who live with behavioral issues. We’ve been successful in decreasing referral delay from primary care (PC) to mental health (MH) services by collaborating on care plans in a way that would not otherwise be possible.
