Alternatives For Youth, Aftercare Service - AFY

Program Purpose:
The goal of Family Service League’s AFY Family Support Program is to promote long-term stability of identified youth and their families in order to prevent involvement with the juvenile justice system.  After participating in EAC’s AFY program, youth and their families are referred to Family Service League for Aftercare services.  Youth are eligible to participate in services for up to one year.   In addition to working with identified youth, Family Service League incorporates services to the entire family system to facilitate long term stability of each participating youth. 

Program Description:
Case management services provide a variety of home visiting and additional support services to identified youth and their families. These services include, but are not limited to, weekly home visits with youth and their families, consistent contact with the schools of each participating youth,  referrals to a variety of counseling and family support services, one to one anger management program with all youth and their families, evening and weekend activities designed to foster youth empowerment and positive behaviors, goal planning and implementation with each participating youth, transportation and concrete assistance as needed.

Persons Served:
The AFY Family Support Program serves adolescents and their families throughout Suffolk County. While there is no minimum age for a child to be enrolled into the program, when the adolescent reaches 18, the family will be referred to other services, if desired.

none

Program Hours:
Families are visited in their homes at mutually convenient times. The program hours are 9-5 Mon-Fri. Scheduling is flexible however, based on the needs of the family. Case managers may visit the child during school hours.
