Program Purpose:
The purpose of this program is to provide Blended, Shelter and Adult Home Initiative Case Management Services for adults with severe Mental Illness including MICA (Mentally Ill Chemical Abusers). Recovery and Strength’s Based Practices are utilized.
Program Description:
Provides OMH Community Based - Case Management Services for individuals in need of assistance in obtaining mental health services, health services, substance abuse services, housing, vocational services, educational services, benefits, legal services and life skills services. Linkage to crisis services when needed. Coordination of services with all community providers assist the consumer in obtaining services and resources. Staffing includes Intensive Case Managers, Supportive Case Managers and Peer Specialists who primarily provide services in the consumers’ home environment.
Persons Served:
Adults – ages 18 and over, with a major Psychiatric or MICA diagnosis who are in need of linkage, coordination and support .
Consumers live in Western Suffolk County.
Fees:
Medicaid and OMH funded program.
Program Hours:
General hours – 8:30am-4:30pm Monday thru Friday
24/7 on-call services
