Assertive Community Treatment (ACT)

Program Purpose:
The purpose of the ACT Team (Assertive Community Treatment Team) is to deliver comprehensive and effective therapeutic, rehabilitative and supportive services to individuals diagnosed with severe mental illness whose needs have not been well met by more traditional service delivery systems.

Program Description:
The Family Service League ACT Team provides an integrated set of evidence-based treatment, rehabilitative, and supportive services via a mobile, multi-disciplinary mental health treatment team. The services provided by the ACT Team include a full range of clinical treatment, psychosocial rehabilitation, and community support services designed to promote recovery by improving psychiatric symptoms, preventing relapse, teaching skills, providing direct assistance and securing community resources necessary for successful functioning in work, school, home, and social relationships.

Persons Served:
The ACT Team provides services for adults who have a severe and persistent mental illness, as listed in the diagnostic nomenclature (current diagnosis per DSM IV), that seriously impairs their functioning in the community.  All referrals for ACT Team services are triaged through the Suffolk County CAMERA unit.

Fees:
The ACT Team services are billable through Medicaid. 

Program Hours:
Monday through Friday from 9AM to 5PM.  All our consumers can reach us
after hours on an emergency pager system.
