Community Health Care Collaborative (CHCC)

Program Purpose:
The Community Health Care Collaborative (CHCC) has been designed to provide community - based medical services and care management at the Iovino South Shore Family Center in collaboration with North Shore LIJ @ Southside Hospital.  By providing these community services, unnecessary   utilization of the Emergency Department will be reduced.

Program Description:
CHCC provides   outreach, linkage and coordination to NSLIJ primary medical care services located at Iovino South Shore Family Center.   Individuals are given medical evaluation and referrals from the medical staff of NSLIJ.  Individuals are then provided Care Management through   Iovino  South Shore Family Center to ensure that referrals are successfully completed.  

Persons Served:
Individuals with complex medical, mental health and /or substance abuse needs and have a history of frequenting the emergency room for non-emergency medical needs.  Individuals frequently have emergent health issues, but do not have a primary care physician.

Fees:
Medicaid and other Medical insurances accepted. Sliding scale available,

Monday through Friday – 9am to 5pm
After-hour coverage
