Program Purpose:
Stepping Stones East Continuing Day Treatment Program helps adults with serious and persistent mental illness to improve functioning in their personal lives, and in the community. The program fosters self-responsibility for the development and maintenance of personal recovery, rehabilitation and wellness. Our program is committed to high-risk persons and individuals with a history of frequent hospitalizations.
Program Description:
The Stepping Stones East program serves a diverse population of adults, 18 years of age and older. Daily activities and therapy are geared to the needs presented by the individual. Services may be transitional or ongoing, and members are encouraged to participate in the formulation and implementation of their own treatment plans. Members are encouraged to participate in a broad range of treatment modalities including Dialectic Behavioral Therapy, Wellness Self Management, Medication Therapy, Medication Education, and Art Therapy. Members also have access to individual counseling on an as needed basis. We also offer a broad range of activity therapy, including our Community Contact Program.
Persons Served:
Adults, 18 years of age and older who are diagnosed with a severe and persistent mental illness. Members may be self referred, referred by families, referred from community residences, referred by psychiatric hospitals, referred by psychiatrists or outpatient clinics.
Fees:
Medicaid, Medicare and sliding scale are available.
