Supportive Case Management For Children and Adolescents (SCM)

Program Purpose:
The purpose of the program is to provide case management services to children, adolescents and their families in order to maximize each youth’s potential for growth and emotional stability within their natural support system.
Program Description:
Supportive Case Management includes an assessment of the child/adolescents strengths and needs and development of a service plan focused on mental health, health, education, recreation, self care and various other areas related to the young person’s functioning. Services include linkage, coordination of services, advocacy and education. Each participant will have a minimum of two visits per month as well as telephone contacts when needed. Case Managers will provide services in the home, school or other community setting.
Persons Served:
Eligible persons are children and adolescents 5-17 years old who reside in Suffolk County and have been diagnosed with a serious emotional disturbance. (Eligibility and referral to Supportive Case Management is done through the Single Point of Access program.)
Fees:
There are no fees charged to participants. Medicaid recipients will have
their services paid for by New York State Medicaid.
Program Hours:
Monday – Friday 9:00 a.m. to 5:00 p.m.
Evening appointments are available as needed for working families.
24 hour emergency coverage is provided through an answering service.
