Program Purpose:
Program Home helps to address the urgent service needs of homeless families who have been placed in emergency shelter facilities by the Department of Social Services and formerly homeless families living in Suffolk County.
Program Description:
Program Home provides housing relocation services to homeless families referred by the Department of Social Services. These services include: transportation assistance to view potential housing; access to real estate listings; assistance with completing DSS paperwork required for relocation; advocacy with prospective brokers; pre-inspection of potential housing to ensure compliance with DSS regulations; assistance in securing donated furniture and household items, as necessary. Program Home also provides home based case management services to formerly homeless families living in Suffolk County for up to a year upon relocation into permanent housing.
Persons Served:
Homeless families referred by the Dept. of Social Services, and formerly homeless families residing in Suffolk County.
Fees:
No fees are charged.
Program Hours:
Monday – Friday 9:00am- 5:00pm
(Hours are flexible based on client need)
