Program Purpose:
The Self-Sufficiency Project provides case management, housing
relocation and support services to homeless individuals and/or families
placed in emergency housing.
Program Description:
Clients receive a variety of services from Family Service League staff
which result in client stabilization and the increased ability to access
necessary resources and benefits. Clients receive the following services
as needed: Intake and Assessment, Crisis Intervention, Housing Relocation
Assistance, Information and Referral, Basic Needs Assistance, and
Transportation Assistance.
A Case Manager will assess the client's mental and physical health,
social functioning, and other immediate needs, as well as identify any
barriers to accessing services and benefits, and will work with each client
to address these issues.
Persons Served:
Homeless individuals and families residing in an emergency housing facility
recognized by the Suffolk County Department of Social Services.
Fees:
None
Program Hours:
Most services are provided during regular business hours although some
evenings and early morning services are available.
