Program Purpose:
Elder-Link is a collaborative project of Family Service League and the Village of Patchogue that assists frail, isolated elderly seniors to help them remain in their homes as long as possible.
Program Description:
Elder-Link provides the elderly client and/or family member with information and resources that help the senior live safely in the community.
A geriatric case manager can provide in-home assessments and connects clients to programs that help elderly low income individuals stay safe, and healthy.
Persons Served:
The services of Elder-Link are available to residents of the Village of Patchogue who are low income and over the age of 60.
Fees:
All services are free of charge.
