Program Purpose:
Elder-Link is a collaborative project of Family Service League, Religious Communities, and the Town of Huntington that assists frail, isolated elderly seniors to help them remain in their homes as long as possible.

Program Description:
Elder-Link provides the client and or family member with information and access to services in the community that will help them maintain their lifestyle at home.
A geriatric social worker provides in-home assessments and helps client to plan for their future. The focus is on the physical, mental, emotional, and spiritual lifestyle changes that support successful aging.

Persons Served:
The services of Elder-Link are available to residents of the Town of Huntington who are over the age of 60.

Fees:
All services are free of charge.

Tuesdays/Fridays: 9:00am-1:00pm
