Program Purpose:
To offer persons with disabilities and senior citizens an opportunity to live in the community.
Program Description:
The NHTD Program offers a wide range of special support services that will help people in their daily lives. These services include re-teaching basic skills, environmental modifications to provide accessibility and safety to homes, community transitional services and intensive service coordination
Persons Served:
The program is for people who choose to live in the community instead of a nursing home and who meet these requirements: 18 years of age or older; Eligible for nursing home level of care; Eligible for Medicaid coverage for community-based long term care.
Fees:
None. Must have active Medicaid
