Ombudservice (Long-Term Care) Program

Program Purpose:
The Long Term Care Ombudsman Program is a resident focused advocacy program working to improve the quality of life for residents living in licensed long term care facilities which includes skilled nursing centers, assisted living and adult homes and family type homes in Suffolk County.
Program Description:
Trained volunteer Ombudspersons visit residents in skilled nursing center, adult homes, assisted living facilities and family type homes on a weekly basis to assure residents’ rights are protected and residents’ receive the care and services to maintain quality of life. They work with residents, families and facility staff to resolve residents’ complaints and provide linkages to resources. Ombudspersons also work to effect change on a state and federal level to improve the long term care system.
Persons Served:
Ombudspersons provide advocacy services to all residents living in licensed long term care facilities in Suffolk County.
Fees:
There is no charge for this service.
Program Hours:
Huntington - Monday through Friday from 9:00 AM to 5:00 PM.
Riverhead – Wednesday from 9:00 AM to 2:00 PM
