Social Care Network (SCN)

The Social Care Network assists eligible Medicaid members with linkage to essential resources that strengthen independence and stability.  SCN Case managers work with individuals to assess Health-Related Social Needs and navigate them to programs that may help with needs such as food insecurity, housing, transportation, utilities and other support services.  Eligible Medicaid Managed Care members receive services at no cost.

Before you begin, please have your Medicaid number ready, if possible. Enter your name exactly as it appears on your Medicaid card.

After you submit the form, a Family Service League SCN Case Manager will review your answers and call you within 5 days to discuss how we may be able to help.
