COTI helps individuals struggling with addiction and their loved ones, and provides comprehensive substance abuse and mental health assessments, diagnosis and medication management in coordination with the development of a treatment plan. The mobile recovery team works with individuals, in the community or in their homes, to address a wide spectrum of needs including medical, housing, financial, employment, legal and family issues. Recovery peer advocates are at the core of this program as they use their experiences to inspire, connect and support individuals struggling with addiction.

CHCC provides community-based medical services and care management for individuals with complex medical, mental health and/or substance abuse needs who have a history of frequenting the emergency room for nonemergency medical needs. Medical evaluations are performed by Northwell Health staff and referrals are made to specialty care, as needed. DASH is a 24/7 voluntary program for individuals in a crisis situation who are feeling overwhelmed due to substance use, mental illness and/or other life stressors.

The program features a mobile response team of licensed professional experts who will come to your community and conduct an assessment, develop a safety plan, and if necessary, recommend outpatient services or a higher level of care.

DASH also features a crisis hotline providing telephone triage, clinical assessments and therapeutic support.

Families need help immediately after a suicide.  Family Service League’s Joe’s Project provides individual, family, and group counseling, as well as support to organizations and communities affected by the loss of someone who died by suicide.  This unique suicide response program has four components:

  • Outreach to educate community members, first responders, and other referral agencies
  • Immediate response by a trained professional to support the newly bereaved
  • Individual, family, and group counseling
  • Suicide prevention education to organizations, schools, and communities

Have you experienced the severe traumatic loss of a loved one to suicide?
You are not alone. FSL offers hope and help for loved ones left behind.

Surviving suicide loss is possible.  Family and loved ones who meet with a counselor often feel less alone and less isolated.  Upon request, FSL’s trained professional staff will visit your home, school, business, or another location of your choosing to help you walk through the excruciating days and weeks that follow your loss.  If you prefer to wait, assistance from a Joe’s Project counselor may be arranged anytime you feel ready.  Our team may also provide or recommend resources for follow up counseling, support groups, and referrals for additional services.

There is HOPE after the traumatic loss of a loved one to suicide.

FSL’s Community Crisis Action Team (C-CAT) responds to traumatic incidents, assessing the needs of individuals and groups who have experienced a critical event.  Such incidents can happen in the workplace, the school, or in the community. Before the creation of Joe’s Project, there was no organized response in Suffolk County to assist those who had lost family or friends to suicide. This program was designed to fill this void.  Joe’s Project is made possible by a grant from the Suffolk County Legislature.  Joe’s Project is named in memory of Joe Maiorella, who died by suicide.

Our clinics and satellites offer professional clinical services to individuals and families in order to reduce mental health symptoms, strengthen and build self-sufficiency and improve quality of life. Treatment often includes a combination of evidenced-based psychotherapy, pharmacological interventions and supportive services. Licensed therapists provide individual, group or family therapy modalities, typically on a weekly or biweekly basis. Licensed prescribers usually meet with clients who receive medications on a monthly or bimonthly basis to review those medications and progress. The clinics accept Medicaid and Medicare as well as most private insurance.

The Certified Community Behavioral Health Clinic (CCBHC) model addresses longstanding challenges that have led to a crisis in  accessing mental health and addiction care.

CCBHCs provide a comprehensive array of services needed to create access, stabilize people in crisis, and provide the necessary treatment for those with mental health and substance use disorders. CCBHCs integrate additional services to ensure an approach to health care that emphasizes recovery, wellness, trauma-informed care, and physical-behavioral health integration. These services include, but are not limited to:

-24/7/365 mobile crisis team services to help people stabilize in the most clinically appropriate, least restrictive, least traumatizing, and most cost-effective settings.

-Immediate screening and risk assessment for mental health, addictions, and basic primary care needs to address the chronic conditions that drive poor health outcomes.

-Easy access to care with criteria to assure a reduced wait time so those who need services can receive them when they need them, regardless of ability to pay or location of residence.

-Tailored care for active duty military and veterans to ensure they receive the unique health support essential to their treatment.

-Expanded care coordination with local hospitals, other health care providers, social service providers, and law enforcement, with a focus on whole health and comprehensive access to a full range of medical, behavioral and supportive services.

-Commitment to peers and family, recognizing that their involvement is essential for recovery and should be fully integrated into care
