Compliance Incident Report

To file a formal complaint about a compliance matter or if you have questions regarding any of the policies above, please call FSL’s Compliance Hotline:

Please note, if your complaint concerns a grievance, for example, a complaint about your services, a staff member or one of our facilities, that is a grievance and should be submitted by using the Grievance Reporting Form. To learn more about what is considered a grievance, click on the link.

Some examples of a Compliance incident are billing for Medicaid services that were not provided, accepting kickbacks (e.g., money) for client referrals or billing for unnecessary services.  Calls to the Hotline are responded to within three business days of receiving them.  You are not obligated to provide your name.  All reports made to FSL are confidential.  However, based on the need to investigate a matter, this may not always be possible.  FSL will not retaliate against anyone for making a good faith report.
